Datasheet for the computing of the probability to be demented.

Presentation

The objective of this datasheet is to propose a screening instrument for dementia based on a reduced number of neuropsychological tests. Seven neuropsychological tests and the MMSE subscores were considered. The most discriminant combination of tests included the MMSE and the subscores "orientation to time" and "recall three objects," the Benton Visual Retention Test, and Isaacs' Set Test of verbal fluency.

Methodology

To compute the probability to be demented, the following variables are needed: 

  • the Mini-Mental State Examination score (MMSE) and the subscores “orientation to time” (TIME) and “delayed recall” (RECALL)
  • the Benton Visual Retention Test (BVRT),
  • and the four subscores of the Isaacs Set Test (IST) which are used only to compute the IST total score.

If these tests are missing, the corresponding missing indicators (Immse, Ibvrt, Iist) must be set to 1.

Then, the probability to be demented is computed by using the formula:

u = 7.398 - 0.070xMMSE - 0.898xTIME - 0.70xRECALL - 5.14xImmse - 0.24xBVRT + 1.3xIbvrt – 0.17xIST + 1.08xIist

Pr(demented) =

If Pr(demented) is > 0.005 then the subject is screened positive.


Using the datasheet

Scoring of MMSE: missing responses for the 5 serial subtractions have to be scored 0.
If the number of missing responses for the 25 other items is greater than two, the total MMSE score has to be considered as missing.
If the number of missing responses for the 25 other items is less or equal than 2, these missing responses have to be scored 0 and the total MMSE score must be computed.
- For each test, the missing indicator must be set to 1 if the score is missing and 0 otherwise.
If the missing indicator is 1, the corresponding scores and subscores have not to be completed because, whatever the values in these fields, default values are used for computation.
- When the missing indicator is 0, the corresponding scores must be completed (in this case, an empty field is considered to be 0). The four subscores of the IST must be completed and the total score is computed by the programme as
IST = min(10,color) + min(10,animal) + min(10,town) + min(10,fruit).
- The probability to be demented is evaluated as soon as a new field is completed and validated.
